Assumption of Risk

Laser tag involves physical activity and may include slipping, tripping, falling, or colliding with other players or inflatable arena structures. By completing this form, I voluntarily accept these risks for myself or my child.

Mobile & Inflatable Arena Notice

Supernova Laser Tag operates a mobile, inflatable arena that may be set up on grass, turf, pavement, or other outdoor surfaces. Natural terrain, weather, and movement inside the inflatable may affect gameplay.

Release of Liability

I release Campfire Entertainment LLP / Supernova Laser Tag, including its owners, employees, and contractors, from any claims or liability for injuries or damages that may occur during participation, to the fullest extent permitted by law.

Safety Rules

I agree that all participants will follow staff instructions and safety rules, including:

• No climbing on the inflatable, other than in the sniper nests

• No roughhousing or physical contact

• No diving or intentional falls

• Proper use of equipment at all times

Health Statement

I confirm that the participant is physically able to play and has no medical conditions that would make participation unsafe.

Photo/Video Permission

I understand that Supernova Laser Tag frequently captures photos and video during games. I grant permission for these images and recordings to be used for promotional and marketing purposes, including social media and advertising, without compensation.

Digital Signature

By submitting this form, I confirm that I am at least 18 years old and legally able to agree to this waiver.
